Advance Care Planning (ACP) has been a billable service since January 1, 2016. It is not about getting a form filled out and put in a file. This conversation may eventually lead to the documentation of the patient’s wishes – but it does not stop there. It is a dynamic conversation that occurs several times with patients and should result in the patient having conversations with their loved ones. Are you doing advance care planning? Are you preparing your patients to have these conversations with their loved ones? Do you offer assistance to them? If you are struggling with these questions or maybe never thought about what happens next, join us for this webinar to gain a greater understanding of how you can be the link between patients and families for these valuable conversations.
Upon completion of the webinar, the participant will understand:
- The various approaches to the conversation and be able to describe the ACP process for the advance directive product
- How to offer guidance to patients to make their wishes known to families and loved ones
The reimbursement opportunities for providing advance care planning services
