Advance Care Planning (ACP) has been a billable service since January 1, 2016. Not every clinical practitioner is qualified to have these conversations by virtue of title or position. Obtaining the skills to have these conversations is not only required for billing, but utilizing these skills along with the appropriate tools and resources will improve the effectiveness of the conversation.

1. The various approaches to the conversation and be able to describe the ACP process for the advance directive product
2. The similarities and differences in the advance directive products/tools
3. The reimbursement opportunities for providing ACP through a team-based approach

of the wellness prevention plan, patients receive the service free of charge.
