The CHNA assists organizations in focusing on those social determinants of health that can make a difference to their community members. In addition to the data that is collected while consumers are patients, additional data on social conditions that affect health and potential disparities in service delivery provide comprehensive information to a service providers.
Upon completion of the webinar, participants will be able to:
1) Understand the relationship between how population groups experience “place” and the impact of “place” on social determinants of health
2) Define the variety of elements that relate to or impact social health, health inequities and quality of life.
3) Identify 2 strategies health care organizations can utilize to impact population health.
