Course Description:
In 2011, Medicare established a new benefit for Medicare Beneficiaries – The Annual Wellness Visit. The Annual Wellness Visit was implemented to focus on health promotion, disease detection, and fostering the coordination of the screening and preventive services for the Medicare Beneficiaries. The Annual Wellness Visit is often misunderstood by providers and patients alike. It is not intended to be an annual physical exam but rather an in-depth consultative visit with the outcome of providing an individualized plan of care for the patient focusing on health and prevention. Additionally, this visit can and should be performed using the team based approach to care.
Now more than ever, it is imperative that healthcare professionals embrace the team based approach to care that encourages everyone to work at the highest level of their education and training. Using a team based approach to care to focus on health and wellness increases access to care, assists patients in making healthy choices, and improves practice workflow. The team based approach is the only effective means of delivering this Annual Wellness Visit benefit to the growing number of Medicare beneficiaries – currently over 55 million with an additional 10,000 plus added each day.
This course is designed to provide health professionals, such as Nurses, Health Educators, Health Coaches, and alike, with the knowledge, skills, and abilities to conduct this in-depth consultative visit and develop an individualized preventive plan of care as a valuable member of the primary care provider team.
Course Delivery Format:
The course is divided into five (5) modules. Each module will run for two (2) weeks. The development of annual wellness visit (AWV) skills occurs when knowledge and practice are combined. This Annual Wellness Visit Fundamentals Certificate Course will put knowledge into practice as participants are immersed in this interactive course. The education will be delivered in a virtual environment using a combination of pre-recorded didactic presentations, asynchronous discussion threads, and a virtual meeting space for a live discussion at the end of each module.

Course Objectives:
At the completion of this course, the participant will be able to:

- Explain the value of the using the Team Based Approached for AWVs
- Select appropriate questions for the health risk assessment
- Translate the data in the medical record into information that impacts the health and wellness of the patient

Instructors:
Cynthia DuBois, BSN, RN

Cyndi DuBois began her nursing career in Pennsylvania over 25 years ago in a large teaching hospital as an adult intensive care and shock trauma nurse. She then transitioned into home health care as a field nurse and progressed to the director of nursing position where she achieved the initial Joint Commission Accreditation for the agency. Her next step was 2000 miles away when she and her husband relocated to Forsyth, MT, his hometown. She began as a staff nurse at Rosebud Health Care Center then transferred to the Family Practice Clinic in Forsyth as a clinic nurse. She continued to work at Rosebud Health Care Center for eighteen years and had responsibilities in care transitions and discharge planning. She is certified in advance care planning and as a Lean Instructor. Currently, she practices as an independent contractor, fulfilling roles in case management, utilization review, chronic care management, annual wellness visits and advance care planning.
Faith M Jones, MSN, RN, NEA-BC
Director of Care Coordination and Lean Consulting
Faith began her healthcare career in the US Navy over 30 years ago. She has worked in a variety of roles in clinical practice, education, management, administration, consulting, and healthcare compliance. Her knowledge and experience spans various settings including ambulance, clinics, hospitals, home care, and long term care. In her leadership roles she has been responsible for operational leadership for all clinical functions including multiple nursing specialties, pharmacy, laboratory, imaging, nutrition, therapies, as well as administrative functions related to quality management, case management, medical staff credentialing, staff education, and corporate compliance. She currently implements care coordination programs focusing on the Medicare population and teaches care coordination concepts nationally. She also holds a Green Belt in Healthcare and is a Certified Lean Instructor.
Continuing Education Information:
HealthTech Management Services D/B/A HealthTechS3 is a Provider approved by the California Board of Registered Nursing. Provider Number CEP8769 for 12.5 contact hours. Participant must complete all course work – no partial credit will be given. A certificate of completion will be provided by HealthTechS3 within 30 days of successful completion of the course.
Refund / Cancellation Policy:
Cancellations received in writing prior to the start of the course will be refunded, less $50.00 administrative expense. Registration fees may be transferred to another individual prior to the start of the educational offering. Individuals who enroll in an online educational course and who open any module content will not be eligible for refunds.
