As our population ages, we will not have the capacity to care for patients in the future using the same care models of the past. More emphasis is being placed on population health and ensuring all care delivered meets the quadruple aim. As payment models embrace team based care, all staff must be prepared to play a crucial role in care delivery. Tracking and ensuring the delivery of annual wellness services for all Medicare beneficiaries ensures sustainable revenue for the practice; creates a measurable baseline of care; empowers patient participation in disease prevention; creates community engagement for healthy communities; and provides physicians with much needed support. The end result is better health for the population, better care for the individual, lower costs for the healthcare system, and improved healthcare provider satisfaction.
Upon completion of the webinar, the participant will be able to:
- Explain team based care as related to new payment methodologies including annual wellness visits
- Understand the elements required in the annual wellness visit and the individualized prevention plan of care
- Identify opportunities and support tools to incorporate clinical and non-clinical community resources into the patient’s experience
