Chronic Care Management began in 2015 and was targeted for primary care practices.  Over the last few years, the success of this Care Coordination model has grown, and specialty practices have expressed interest in obtaining reimbursement for the care coordination they provide.  Medicare listened!  Beginning in 2020, a new care coordination model targeted for specialty practices has emerged.  Join us for this webinar to review the basics of chronic care management, learn about principle care management and understand how the programs complement and intersect.
