Care Coordination: Adding Behavioral Health to your Chronic Care Management Program

Hosts: Faith M Jones, MSN, RN, NEA-BC HealthTechS3 Director of Care Coordination and Lean Consulting

Date: June 28th, 2018

Time: 12:00pm CT

Behavioral health issues are found in all segments of the populations and often go undetected in our older population.  For example, almost one third of all older adults are impacted by depression. Including these patients and others with behavioral health issues in a care coordination program can make a positive impact on their lives.

In 2018, care coordination in the form of behavior health integration (BHI) became a reimbursable service for primary care practices.  If you already have a chronic care management program, the inclusion of BHI is a simple and logical next step.  Join us for this webinar for an overview of the rules for BHI and how it aligns with Chronic Care Management to create a sustainable care delivery model with a positive impact on your patient’s lives and your practice’s financial health.

Upon completion of the webinar, the participant will understand:

1. The elements of a Behavioral Health Integration Program

2. How BHI aligns with Chronic Care Management

3. The reimbursement methodologies for BHI in various types of clinics
