Every not-for-profit hospital is required to complete a Community Health Needs Assessment (CHNA) every three (3) years. Hospitals are also under increased pressure to improve patient outcomes and reduce healthcare costs. The presentation will focus on the synergy between an organization’s CHNA and its Population Health initiatives.

1. Identify at least two ways that the organization’s CHNA can help to promote population health.
2. Identify how to engage the organization in discussions about population health.
