While in the hospital, healthcare professionals are focused on the immediate needs of the patient and what steps are necessary to promote healing. In our fast-paced acute care settings, this does make some sense, but as we move more and more into a value-based world the focus must be ensuring the patient and family have everything they need to manage at home after discharge.
