Advance Care Planning (ACP) has been a billable service since January 1, 2016. Not every clinical practitioner is qualified to have these conversations by virtue of title or position. Obtaining the skills to have these conversations is not only required for billing, utilizing the appropriate tools and resources will improve the effectiveness of the conversation. ACP is one of the procedures allowable under a team-based care approach and should be incorporated as part of the wellness prevention plan.
