The 2021 Physician Fee Schedules has small changes for care coordination but some big changes for clinic visit documentation in general.  As pre-visit planning is an integral part of care coordination that embraces team-based care and assists with the efficiencies of the visit, this and other workflow updates can be a big win in practice documentation.  As of 2021, “the new rules have gotten rid of the history and exam requirement for determining level of visit.
