The Centers for Medicare and Medicaid Services has established a standard of Patient Centered Care for both acute and critical access hospitals which is a step in the right direction, yet aren’t we leaving out some important parties in assuring the necessary transformation of care and service delivery? Similar to the team approach that is encouraged in all types of organizations, it is essential that patients are engaged in their care, AND that those who are family care givers are also integrated into the plan of care as a means of providing optimal outcomes.
