On September 30, 2019, CMS issued major regulatory changes for hospitals and critical access hospitals. The new regulations require additional patient information be provided to the next care provider as well as requiring that the hospital must assist patients in selecting a post-acute care (PAC) services provider by using and sharing PAC data on quality measures and resource use measures.
Although simple on the surface, the details are always a little more complicated.
This webinar will focus on understanding the new regulations as well as offering some practical tips for meeting the new requirements.
- Describe the new discharge planning requirements
- Understand how to access and use quality measures as part of the discharge planning process
