A critical component of population health management is the ability to easily collect and analyze data to affect positive outcomes for the individuals who will be managed.  Organizations generally collect volumes of data that can be used to make decisions regarding how and where care is provided, yet the traditional model of care delivery that has been used for decades is difficult to change, as are the behaviors of those who provide care.
