The Annual Wellness Visit (AWV) and Advance Care Planning (ACP) are both services recognized by the Centers for Medicare and Medicaid Services (CMS) as Team-Based Care Services. Using the right person with the right training to perform these services leads to increased patient engagement, improved access to care, and increased revenue in a clinical practice.
